About The Position

Amedisys is hiring a full-time Registered Nurse hospice case manager for the first shift, serving the Prince George County territory. The RN case manager provides hospice care to patients in their homes, where they are most comfortable. The role includes assessing patients, coordinating and implementing individualized plans of care, maintaining patient records, supporting symptom management, educating patients and caregivers, and communicating with the care team. On-call responsibilities are assigned.

The position offers an annual salary range of $98,000 to $105,000. Benefits may vary by position and shift status, and medical-plan-related benefits require enrollment in an Amedisys medical plan.

Requirements

• One year of nursing experience.
• Current RN license specific to the state or states assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents the rationale for not doing so.
• Develops, prepares, and maintains accurate, timely, individualized patient care progress records according to care center policies.
• Records pain and symptom management changes and outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with the clinical manager and care team.
• Performs assigned on-call responsibilities and provides on-call services to patients and families.
• Performs other duties as assigned.
