About The Position

Aspire Home Healthcare is seeking Registered Nurses to provide one-on-one home health care that helps patients live comfortably and safely in their homes. The RN provides direct patient care and case management in accordance with physician orders, including patient admissions, care-plan development, and coordination with the interdisciplinary team. Opportunities are available in Ross County, Ohio, with full-time and part-time options. The role is paid per visit, with rates starting at $55 per visit, and involves travel to patients’ homes. The position is listed as part-time, with a day shift. Orientation must be attended at the Dublin office.

Requirements

• Active RN license in Ohio.
• Current TB test completed within the last 12 months.
• Current CPR certification.
• Valid driver’s license.
• Must be able to attend orientation at the Dublin office.

Nice To Haves

• Experience with OASIS and WellSky (Kinnser) is strongly preferred.
• Med-surg and wound care experience is a plus.

Responsibilities

• Obtain patients’ medical histories and assess their conditions and needs.
• Conduct physical assessments, including vital-sign and mobility evaluations.
• Educate patients and families about care plans and home health services.
• Deliver skilled nursing care while maintaining HIPAA compliance.
• Document and submit accurate visit records promptly.
• Develop and update individualized care plans as needed.
• Coordinate care with LPNs, Home Health Aides, and Personal Care Attendants.
• Facilitate referrals for additional services, such as therapy or medical social work.
• Evaluate the effectiveness of nursing services and adjust care as appropriate.
• Provide direct patient care and case management in accordance with physician orders.
