About The Position

Elara Caring is hiring a Registered Nurse Home Health to provide compassionate nursing care to patients in their homes. The nurse assesses patient needs, completes initial and ongoing assessments, and provides care according to the plan of care and physician orders. The role also involves protecting patient and staff privacy in accordance with HIPAA, monitoring patient conditions, reporting changes to the care team, educating patients and families, and taking appropriate action when conditions change. The nurse helps ensure quality, safe, and continuous patient care.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• At least 1 year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred, but not required.
