About The Position

This position is for a Registered Nurse in Home Health Case Management, serving the North Central Suburbs. The role involves developing and maintaining therapeutic relationships with patients and families, providing individualized care, and mobilizing resources to ensure quality, cost-effective care. The nurse will also be involved in patient and staff education, utilizing the nursing process for problem-solving, and demonstrating leadership and collaboration within the healthcare team. This is a full-time position with a standard Monday-Friday schedule, including rotating weekend and on-call duties.

Requirements

• Graduate from an accredited school of nursing
• One year of medical surgical experience
• Has as demonstrated proficiency for the requirements for RN role
• Good communication and interpersonal skills
• Good documentation skills.
• Computer skills
• Good time management and organizational skills
• Familiarity with patient care equipment
• Current Illinois RN license
• Current CPR
• Nurse, Registered (RN)
• A valid driver’s license issued by the Division of Motor Vehicles.
