About The Position

BAYADA Home Health Care is seeking a Registered Nurse (RN) to join their team in Flagstaff. The role involves providing 1:1 skilled nursing and rehabilitative care to clients in their homes, enabling them to remain at home. This is a per diem opportunity with a flexible schedule and requires participation in a weekend on-call rotation.

Requirements

A current nursing license in good standing in the state of Arizona
Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
Must have 1 year nursing experience
Prior home health and OASIS experience required.
Taking part in weekend on-call rotation required.

Responsibilities

Deliver 1:1 skilled nursing, intervention, and supportive care in accordance with individual care plan, including administration of medication, wound care and treatments
Perform client assessments, provide education to the patient and/or their family, and monitor patients' condition.
Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
