About The Position

The ACT/FACT team nurse acts as a team player and shares in the responsibility of carrying out rehabilitation and support functions and assisting in treatment, substance abuse services, education, support and consultation to families and crisis intervention services under the clinical supervision of staff with professional degrees. The nurse is specifically responsible for medical and medication monitoring and education for those on the ACT/FACT team.

Requirements

• A current Louisiana Nursing License
• A valid Louisiana driver's license and reliable transportation is required.
• Willing to work in a community setting with members diagnosed with SMIs and Co-Occurring Disorders.

Responsibilities

• Medical and medication monitoring and education for those on the ACT/FACT team.
• Carrying out rehabilitation and support functions.
• Assisting in treatment, substance abuse services, education, support and consultation to families.
• Crisis intervention services under the clinical supervision of staff with professional degrees.
• Travel throughout the parish in a community-based setting.
