About The Position

Aspire Home Healthcare is seeking Registered Nurses to provide one-on-one home health care in Ross County. The RN provides direct patient care and case management in accordance with physician orders, including patient admissions, care planning, and coordination with the interdisciplinary team. The position offers full-time and part-time opportunities and pays per visit, with rates starting at $55 per visit. Nurses must attend orientation at the company’s Dublin office.

Requirements

• Active Registered Nurse license in Ohio.
• Current TB test from within the last 12 months.
• Current CPR certification.
• Valid driver’s license.
• Ability to attend orientation at the Dublin office.

Nice To Haves

• Experience with OASIS and WellSky (Kinnser) is strongly preferred.
• Med-surg experience is a plus.
• Wound care experience is a plus.

Responsibilities

• Obtain patients’ medical histories and assess their conditions and needs.
• Conduct physical assessments, including vital-sign checks and mobility evaluations.
• Educate patients and families about care plans and home health services.
• Provide skilled nursing care while maintaining HIPAA compliance.
• Document and submit accurate visit records promptly.
• Develop and update individualized care plans as needed.
• Coordinate care with LPNs, Home Health Aides, and Personal Care Attendants.
• Arrange referrals for additional services, such as therapy or medical social work.
• Evaluate the effectiveness of nursing services and adjust care as appropriate.
