About The Position

Advanced Care Partners is seeking a Registered Nurse (RN) to provide individualized care for medically fragile children and adults in home and school settings. This position offers a more personalized care model, allowing for stronger patient outcomes and professional satisfaction. The role involves delivering one-on-one, patient-centered care, developing meaningful relationships with patients and families, and being supported by a collaborative team and 24/7 on-call assistance. It offers increased consistency and a reduced patient load compared to facility settings.

Requirements

• Active RN license in Georgia or Nurse Licensure Compact
• A minimum of one year of RN experience is required.
• Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or an equivalent accredited organization required.
• Valid PPD skin test

Nice To Haves

• Vent experience preferred

Responsibilities

• Provide skilled nursing care (feeding tubes, tracheostomies, ventilators, central lines)
• Monitor patient progress and respond to changes in condition
• Educate patients and families on treatment plans
• Maintain a safe and clean care environment

Benefits

• Base Pay: $45/hour
