About The Position

Amedisys is hiring a PRN registered nurse to coordinate and oversee patients’ home care. The nurse works with patient care teams to develop and implement care plans, educates families, assesses patients, provides and manages clinical treatments, and communicates patient progress to the clinical manager, physician, and care team. The role also supervises LPNs and home health aides and requires timely, accurate documentation.

Requirements

• At least one year of clinical experience as a registered nurse; candidates with less than one year must be approved by the VP of Clinical.
• Current RN license applicable to the state or states where assigned to work.
• At least six months of home health experience.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and liability insurance.
• Ability to assess patient status and identify requirements related to age-specific needs.
• Familiarity with general computer use and functions, including usernames and passwords, internet and email, desktop navigation, programs, files and windows, navigation buttons and toolbars, and online HR services and training programs.
• Candidates with less than six months of clinical experience as an RN must participate in the RN Intern program.

Responsibilities

• Coordinate and oversee all aspects of patients’ home care.
• Collaborate with patient care teams to develop and implement care plans.
• Educate families about patients’ conditions, needs, and care.
• Perform head-to-toe physical assessments.
• Check vital signs and manage medications.
• Administer injections, IV therapy, and tube feedings.
• Administer and maintain catheters.
• Complete documentation accurately and on time.
• Supervise LPNs and home health aides.
• Regularly communicate patient progress to the clinical manager, physician, and care team.
• Perform other duties as assigned.
