About The Position

Attend Home Care is seeking a compassionate and skilled Registered Nurse (RN) who is passionate about delivering high-quality patient care in the home setting. This is a part-time/per-diem role with flexible scheduling available, offering the opportunity to build meaningful patient relationships while growing your career. The role involves conducting comprehensive clinical assessments, developing individualized care plans, performing skilled nursing visits, and providing hands-on care. You will also be responsible for patient and caregiver education, collaborating with healthcare professionals, maintaining accurate documentation, and supervising licensed practical nurses and home health aides.

Requirements

• Active Registered Nurse (RN) license in the state of practice
• Experience in home health, skilled nursing, or a related healthcare setting preferred
• Strong clinical assessment and critical thinking skills
• Knowledge of home health regulations and documentation standards
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively
• A passion for providing compassionate, patient-centered care

Responsibilities

• Conduct Start of Care (SOC) visits to complete comprehensive clinical assessments and develop individualized, physician-directed care plans
• Perform skilled nursing visits and ongoing reassessments to monitor patient condition and adjust care plans as needed
• Provide hands-on skilled nursing care, including wound care, medication management, injections, and chronic disease management
• Deliver patient and caregiver education on diagnoses, medications, and preventive care to reduce hospitalizations
• Collaborate with physicians and interdisciplinary team members to ensure coordinated, high-quality care
• Maintain accurate and timely clinical documentation in compliance with state regulations and agency standards
• Supervise and delegate care to LPNs and Home Health Aides to ensure quality and adherence to the plan of care
