About The Position

The Registered Nurse provides direct nursing care to patients, assists medical providers and integrated pod team members to optimize the patient care experience, and works with the care coordination team. This role is responsible for triage across clinic sites and functions as a nurse leader when assigned to a pod team. Key responsibilities include utilizing professional nursing clinical skills and assessment for primary care, participating in health care team planning for optimal patient care, and managing daily pod patient flow, including nurse visits, medical assistant visits, co-visits, and walk-ins. The nurse will also assist and facilitate patients in obtaining medical services, provide patient triage and assessment, obtain health information, and consistently follow telephone triage guidelines. Further duties involve transitioning care for patients discharged from the hospital to prevent readmission, evaluating appropriate follow-up care for emergency department patients, providing medications and conducting medication reconciliation according to protocol, and ensuring a safe, comfortable, therapeutic environment for patients and families. The role also requires identifying changes in clinical signs and symptoms in life-threatening situations and initiating appropriate measures, evaluating patient care outcomes, consulting with other professionals, and adjusting nursing care processes as necessary. Additionally, the Registered Nurse assists with OSHA, CLIA, VFC, and Cancer Screening Programs, and provides co-visits in collaboration with providers and medical assistants, including initial assessment, documentation, and patient education. Additional job duties may be assigned based on clinic need.

Requirements

  • Bachelor's degree in nursing
  • Current Registered Nurse (RN) license to practice in the state is required.
  • Current Basic Life Support for Healthcare Providers (BLS-HCP) course completion card from the American Heart Association is required
  • Must be current or willing to be vaccinated for the following: TDAP (up to date booster), MMR series, Varicella, HEP B series, TB Screening

Nice To Haves

  • 1+ year RN experience in primary care setting
  • Bilingual in Spanish
  • Nationally Recognized Advanced Certification (i.e. CCRN)

Responsibilities

  • Utilize professional nursing clinical skills and assessment to provide primary care nursing.
  • Participate as member of health care team planning for and providing optimal patient care including establishing nursing care plans when needed.
  • Manage and lead daily pod patient flow, including nurse visits, medical assistant visit, co-visits and walk-ins.
  • Assist and facilitate patients in obtaining medical services, including scheduling patients as appropriate, triaging patients, and assessing patients who walk-in to the clinic seeking services.
  • Provide patient triage and assessment and obtain health information and relevant data.
  • Consistently follows telephone triage guidelines, including documentation and notifying provider in a timely manner.
  • Transition care for patients discharged from the hospital within 24-48 hours to prevent readmission and related complications.
  • Evaluation of appropriate follow-up care for patients seen in the emergency department.
  • According to departmental protocol, provide medications and conduct medication reconciliation.
  • Provide a safe, comfortable, therapeutic environment for patients and families.
  • Identify changes in clinical signs and symptoms in life threatening situations and initiate appropriate measures.
  • Evaluate outcome of patient care, consult with other professionals as required, and adjust nursing care processes as necessary to ensure optimal patient care.
  • Assist with OSHA (Occupational Safety and Health Administration), CLIA, VFC (Vaccines for Children) and the Cancer Screening Programs and other organization or site quality processes.
  • Provide co-visits in collaboration with providers and medical assistants. Responsibilities include initial assessment, documentation, and patient education.
