The Registered Nurse provides direct nursing care to patients, assists medical providers and integrated pod team members to optimize the patient care experience, and works with the care coordination team. This role is responsible for triage across clinic sites and functions as a nurse leader when assigned to a pod team. Key responsibilities include utilizing professional nursing clinical skills and assessment for primary care, participating in health care team planning for optimal patient care, and managing daily pod patient flow, including nurse visits, medical assistant visits, co-visits, and walk-ins. The nurse will also assist and facilitate patients in obtaining medical services, provide patient triage and assessment, obtain health information, and consistently follow telephone triage guidelines. Further duties involve transitioning care for patients discharged from the hospital to prevent readmission, evaluating appropriate follow-up care for emergency department patients, providing medications and conducting medication reconciliation according to protocol, and ensuring a safe, comfortable, therapeutic environment for patients and families. The role also requires identifying changes in clinical signs and symptoms in life-threatening situations and initiating appropriate measures, evaluating patient care outcomes, consulting with other professionals, and adjusting nursing care processes as necessary. Additionally, the Registered Nurse assists with OSHA, CLIA, VFC, and Cancer Screening Programs, and provides co-visits in collaboration with providers and medical assistants, including initial assessment, documentation, and patient education. Additional job duties may be assigned based on clinic need.
