Riverside Hospice - PRN - Registered Nurse
About The Position
Riverside Home Health & Hospice is seeking an experienced PRN Hospice Registered Nurse to provide additional clinical support when patients and the team need it. This role offers a meaningful opportunity to use clinical judgment and compassion to make a difference in end-of-life care. The position is ideal for a nurse who finds purpose in end-of-life care, desires flexibility, is confident in patient assessments and symptom management, thinks holistically, remains calm in sensitive situations, is self-directed, and takes ownership of patient care. Riverside Home Health & Hospice has been serving patients in Southern Oregon for over 20 years, focusing on providing comfort, dignity, and compassionate care during life's sacred moments.
Requirements
- Graduate of an accredited school of nursing
- Active Registered Nurse license in Oregon
- Current CPR certification
- Strong clinical assessment and symptom-management skills
- Excellent communication and documentation abilities
- Ability to organize and prioritize independently
- Ability to work confidently in patients’ homes
- Valid driver’s license and insured, reliable vehicle
Nice To Haves
- Hospice experience strongly preferred
- Homecare Homebase (HCHB) experience preferred
Responsibilities
- Completing scheduled and PRN hospice nursing visits
- Performing comprehensive patient assessments
- Assessing and managing pain and other end-of-life symptoms
- Administering medications and treatments according to physician orders
- Educating patients and caregivers about medications, symptoms, disease progression, and comfort measures
- Supporting patients and families through changes in condition
- Coordinating with physicians, providers, pharmacies, DME companies, and other members of the care team
- Collaborating with the interdisciplinary hospice team
- Completing timely and accurate clinical documentation
- Helping ensure patients have the medications, equipment, supplies, and resources they need
- Providing additional coverage when census, acuity, vacations, or staffing needs require support
