About The Position

Amedisys is hiring a full-time registered nurse case manager to provide home health care. The role supports patients as they heal at home and involves assessing patients, working with the care team to plan and deliver care, promoting health and independence, and supervising LPNs and home health aides. The nurse also completes documentation, communicates patient progress, contributes to staff education and continuing education, and performs assigned on-call services.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP Clinical.
• Current RN license applicable to the state or states of assignment.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement plans of care.
• Refer patients to other disciplines based on identified needs, or document the rationale for not making a referral.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including instruction in self-care techniques.
• Supervise LPNs and home health aides.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
