About The Position

Ascension Medical Group is seeking a Registered Nurse for their Vascular Interventional Radiology and Vein Solutions clinic in Indianapolis, IN. This full-time position involves implementing and monitoring patient care plans, coordinating care among disciplines, carrying out physician and nursing orders, and assessing discharge planning needs. The role requires a strong interest in the dialysis patient population, the ability to work independently and in team settings, and excellent customer service skills.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.

Nice To Haves

Previous experience with the dialysis patient population is highly encouraged.
Special interest in dialysis access centers, dialysis patients, and coordination between providers.
Ability to work independently and within multiple team settings.
Great customer service is a must.

Responsibilities

Implement and monitor patient care plans.
Monitor, record and communicate patient condition as appropriate.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Note and carry out physician and nursing orders.
Assess and coordinate patient's discharge planning needs with members of the healthcare team.
