About The Position
DaVita is seeking an organized, self-directed Registered Nurse to manage Home Hemodialysis (HHD) patients. The role trains patients and caregivers in a clinic setting so they can perform dialysis independently at home. It combines clinical care, patient education, and case management. Home visits are limited, and the clinic is located in Longview, Texas. The schedule is Monday through Friday with eight-hour shifts, and weekends and holidays off. Scheduling is flexible to support managing an individual caseload. The position will be open for a minimum of three days.
Requirements
- RN license in the state of practice.
- Valid driver’s license, insurance, and reliable transportation.
- Associate Degree in Nursing (ADN) required.
- Basic computer skills, including MS Word and Outlook.
Nice To Haves
- Bachelor of Science in Nursing (BSN) preferred.
- Experience with dialysis, including HHD, PD, chronic, or acute dialysis.
- Patient education or teaching background.
- Case management or telehealth experience.
- CNN or CDN certification.
Responsibilities
- Train patients and caregivers on HHD procedures and self-care.
- Manage a caseload of HHD patients, including assessments and care planning.
- Monitor treatment progress, conduct home visits, and provide on-call support.
- Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes.
