About The Position

As a Registered Nurse (RN), Hospice the work you do every day makes a difference in the lives of our patients. Our nurses give our patients the greatest gift – the ability to spend enhanced quality time with their loved ones in their preferred environment. As a RN you will have the opportunity to provide one on one patient care and work at the top of your license. You will utilize your clinical skills in providing end of life care as directed by an attending physician. You'll be part of an interdisciplinary team that focuses on providing compassionate quality care to our patients.

Requirements

  • Registered Nurse with current license in the state of employment.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.

Nice To Haves

  • Minimum one-year experience as an RN in an acute care setting is preferred.
  • Hospice experience is desired.
  • Compassionate communicator with a positive attitude.
  • Patience is a virtue when working with patients, families, physicians, and coworkers.
  • Attention to detail is critical, as is being observant and following directions.

Responsibilities

  • Oversee the overall care of the patient from beginning to end of life.
  • Observe and monitor the patient to determine if the level of care being provided meets the patients’ needs.
  • Work autonomously and be responsible for all practices and duties within the scope of practice outlined by the state.
  • Evaluate the treatment and medical condition of the patient according to the plan of care.
  • Individualize the plan of care and make adjustments as needs change.
  • Update all doctor orders in patient chart and administer medication as prescribed by the physician.
  • Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
  • Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care.
  • Listen to patient and family members.
  • Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Oversee and supervise total care of patient provided by nurse aides and LPNs.
  • Attend in-service training and mandatory agency meetings.
