About The Position
Baptist Health is seeking a Registered Nurse to join their innovative Hospital at Home team. This program allows patients to complete their hospital stay at home, equipped with remote patient monitoring, technology kits, and communication devices. A 24-hour command center, staffed by nurses, virtually monitors patients, with contracted personnel providing in-home care when necessary. This initiative represents a significant transformation in healthcare delivery. As a Registered Nurse, you will be responsible for patient care through the nursing process (assessment, planning, implementation, evaluation), aligning with the organization's mission. You will also organize and supervise care provided by support staff, coordinate with ancillary services, and involve patients and their families in developing individualized care plans.
Requirements
- ASN Required
- Basic Life Support (BLS) Required
- Licensed Registered Nurse Required
Responsibilities
- Education of patients/SO/families to their disease process and plan of care.
- Serving as patient advocate
- Providing care to patients requiring treatment of any medical, surgical, or other condition, including chronic medical and minor injuries or illnesses.
- Organize and supervise care provided by supportive personnel.
- Coordinate care provided by ancillary services.
- Involve patients and significant others (SO)/families in developing an individualized plan of care.
