About The Position

BrightStar Care is seeking a compassionate and experienced Registered Nurse (RN) to join our growing clinical team. Requirements

  • Current unrestricted Ohio Registered Nurse (RN) license
  • Graduate of an accredited nursing program
  • Minimum 3 years of clinical nursing experience
  • Minimum 1 year of case management experience
  • Current CPR certification
  • Valid driver's license
  • Reliable transportation with current auto insurance
  • Ability to travel throughout assigned service areas
  • Ability to pass a background check, drug screen, and other pre-employment requirements

Nice To Haves

  • Home Health experience
  • Pediatric nursing experience
  • Case management experience

Responsibilities

  • Complete comprehensive patient assessments and reassessments
  • Develop and implement individualized Plans of Care
  • Provide skilled nursing services including: Wound care, Medication administration, Injections, Infusions, Catheter care, Other nursing treatments within RN scope of practice
  • Monitor patient conditions and communicate changes promptly
  • Maintain accurate and timely clinical documentation
  • Follow all infection prevention and safety protocols
  • Provide skilled nursing care for pediatric and adult patients
  • Infant pediatric care
  • Seizure monitoring
  • Support patients requiring assistance with Activities of Daily Living (ADLs), including: Bathing, Dressing, Feeding, Toileting, Mobility and transfers
  • Coordinate care with physicians, families, therapists, and interdisciplinary teams
  • Support new client admissions and transitions of care
  • Review patient progress and adjust care plans as needed
  • Ensure compliance with BrightStar Care policies, Joint Commission standards, and state and federal regulations
  • Conduct supervisory visits
  • Complete caregiver competency evaluations
  • Provide clinical education and support to caregivers and nursing staff
  • Assist with orientation and ongoing skills validation
  • Participate in quality assurance and chart review activities
