About The Position

We're seeking a licensed Georgia RN to complete initial and ongoing client nursing assessments for individuals receiving personal care services. This is a flexible, contract-based role, with telehealth available for qualifying visits.

Requirements

• Active, unrestricted Georgia RN license (in good standing)
• Current CPR certification
• Updated TB Test results
• Comfortable using telehealth platforms and EMR/documentation systems
• Reliable transportation (for in-home visits) and reliable internet (for telehealth visits)
• Must pass background check

Nice To Haves

• 1+ year of experience in home care, geriatric, or community health nursing

Responsibilities

• Complete initial nursing assessments for new clients, in-home or via telehealth
• Perform routine 90-day supervisory visits for existing clients to monitor care quality, client satisfaction, and ongoing service needs
• Conduct periodic reassessments in line with state and Medicaid documentation requirements
• Document findings accurately and submit within required timeframes
• Communicate health or care-plan changes to the care support team
• Maintain compliance with Georgia home care regulations and HIPAA
