About The Position
This position is for a Registered Nurse who wants to continue using their clinical knowledge without providing direct or physical patient care. The RN in this role completes in-home visits by talking with clients and families, reviewing health history, and developing clear, practical care plans for non-medical in-home caregivers to follow. The work is focused on assessment, communication, and care planning—not clinical tasks. This role is centered on conversation, observation, documentation, and relationship-based support.
Requirements
- Active Registered Nurse (RN) license
- Ability to complete thorough verbal and observational assessments
- Strong documentation and care-planning skills
- Comfortable working independently in the community
- Professional, compassionate communication style with clients, families, and caregivers

Responsibilities
- Travel to clients’ homes to complete scheduled, non-clinical nursing assessments
- Conduct verbal and observational assessments with clients and family members
- Review medical history, diagnoses, medications, functional abilities, and daily routines
- Identify risks, support needs, and appropriate non-medical services
- Develop and update individualized care plans for in-home caregivers to follow
- Serve as a clinical resource to the care team regarding the client’s plan of care
- Maintain accurate and timely documentation of assessments and care plans
