Responsibilities

• Conduct initial and ongoing assessments, including Start of Care, recertifications, resumptions, and discharges.
• Develop, implement, and evaluate individualized plans of care.
• Administer medications, wound care, and other treatments.
• Educate patients and families about care routines, medications, and disease management.
• Collaborate with physicians, therapists, and the interdisciplinary team.
• Document care accurately in WellSky EMR or a similar system.
• Ensure compliance with Medicare, Medicaid, and agency standards.
• Participate in weekend rotation as needed.
