About The Position

Capitol View Home Health Care is seeking compassionate and clinically strong Registered Nurses (RNs) for PRN Skilled Home Health opportunities throughout Washington, DC. We're looking for nurses who are comfortable working independently, confident in their clinical judgment, and passionate about helping patients safely recover and manage their health at home. Requirements

Active Washington, DC Registered Nurse License Required
Graduate of an accredited nursing program
Strong nursing assessment and clinical decision-making skills
Experience providing skilled nursing care
Strong communication and documentation skills
Excellent organizational and time-management abilities
Ability to work independently in patients' homes
Reliable transportation and ability to travel between assigned patient locations

Home Health and OASIS Experience Preferred

Responsibilities

Perform comprehensive skilled nursing assessments
Complete Start of Care, Resumption of Care, Recertification, and Discharge visits
Complete OASIS assessments as required
Develop and update individualized plans of care
Provide wound care and monitor wound progression
Complete medication reconciliation and provide medication education
Monitor patients for changes in condition
Provide disease-management education
Educate patients, families, and caregivers
Communicate with physicians regarding changes in patient status
Obtain and implement physician orders as appropriate
Coordinate care with therapists, social workers, and other members of the interdisciplinary team
Identify concerns early and help reduce avoidable hospitalizations
Complete accurate and timely clinical documentation
Ensure care is provided according to physician orders, agency policies, and applicable regulations
