The Registered Nurse (RN) will promote wellness, improve outcomes, and support self-care within a defined population of patients, particularly those with chronic conditions like Hereditary Hemorrhagic Telangiectasia (HHT) and Vascular Anomalies. The RN will provide patients with the support, education, and assistance needed for disease prevention and/or maintenance of their health and wellness state. This involves working with individual patients, their families, and providers to develop and implement individual health plans and support patient self-management. The role includes coordinating care and follow-up for patients receiving care in outpatient facilities, managing prescription refill requests, and providing patient education in collaboration with physician staff. A key aspect of this role is ensuring seamless care for patients with HHT and/or Vascular Anomalies, acting as the primary coordinator for the HHT and Vascular Anomalies Program. This involves coordinating diagnostic testing, specialty consultations, treatment plans, and facilitating communication between patients, providers, and multidisciplinary teams. The RN will also provide patient and family education, triage clinical concerns, facilitate timely follow-up, and support complex care coordination. Additionally, the position supports HHT and Vascular Anomalies-related research activities, including patient screening, enrollment, data collection, and regulatory requirements. Strong clinical judgment, communication, organization, care coordination, and proficiency with the electronic health record are essential. The role also contributes to program growth through patient and family engagement, educational initiatives, community and provider outreach, and innovative programs to improve awareness, access, and the patient experience.
