About The Position

Aspire Home Healthcare is seeking Registered Nurses to provide direct patient care and case management to home health patients in the Redding/Red Bluff area. Nurses provide care in accordance with physician orders, conduct patient admissions, develop care plans, and coordinate services with the interdisciplinary team. The role offers full-time, part-time, and per-diem opportunities and pays per visit, with rates starting at $95 per visit. Nurses must attend orientation at the Yuba City office.

Requirements

• Active California RN license.
• Current physical examination completed within the last 6 months.
• Current TB test completed within the last 12 months.
• Current CPR certification.
• Valid driver’s license.
• Must be able to attend orientation at the Yuba City office.

Nice To Haves

• Experience with OASIS and WellSky (Kinnser) is strongly preferred.
• Med-surg and wound care experience is a plus.

Responsibilities

• Obtain patients’ medical histories and assess their conditions and needs.
• Conduct physical assessments, including vital-sign and mobility evaluations.
• Educate patients and families about care plans and home health services.
• Provide skilled nursing care while maintaining HIPAA compliance.
• Document and submit accurate visit records promptly.
• Develop and update individualized care plans as needed.
• Coordinate with LPNs and Home Health Aides.
• Facilitate referrals for additional services, such as therapy or medical social work.
• Evaluate the effectiveness of nursing services and adjust care as appropriate.
