Responsibilities

  • Protect and promote health, prevent illness and injury, and alleviate suffering through nursing diagnosis and treatment.
  • Demonstrate leadership in the professional practice setting through oversight of the interdisciplinary health care team to provide holistic care and advocate for patients and families served.
  • Utilize nursing process - assessed via direct observation, peer reviews, audits, and documentation.
  • Use the results of quality of care/performance improvement activities to initiate changes in nursing practice.
  • Collect clinical data to monitor quality and effectiveness of nursing care.
  • Identify aspects of care that are important for quality monitoring and formulate recommendations to improve nursing practice or patient/resident outcomes.
  • Assist in development and implementation of policies, procedures, and practice guidelines to improve quality of care and nursing practice.
  • Participate on interdisciplinary teams that evaluate clinical practice or health services relating to patient/resident care.
  • Participate in QI studies: involved in task force or committee.
  • Implement suggestions for improving outcomes per unit study results.
  • Independently identify problematic areas and recommend problem solving.
  • Share findings of studies, audits with others.
  • Seek improvement independently.
  • Collaborate and communicate with the patient/resident, family, and other health care providers.
  • Utilize peer review and observation.
  • Monitor documentation for appropriate referrals.
  • Formulate the plan of care and involve patient/residents in decisions related to care and the delivery services.
  • Make referrals, including provisions for continuity of care, as needed.
  • Act as a primary clinical resource for staff on assigned unit.
  • Act as a role model and mentor for staff.
  • Act as the Nurse Manager Representative in his/her absence.
  • Provide leadership and support for staff through effective problem solving and decision making.
  • Provide clinical resource and role model for staff.
  • Support nurse preceptors and new employees during orientation.
  • Provide input on employee evaluations.
  • Assure float staff receive orientation to the floor and feel welcome.
  • Make and adjust staff assignments based on the abilities of staff and needs of the patient population.
  • Assign admissions and arrange transfers in coordination with the Nurse Manager, House Supervisor and/or Bed Coordinator.
  • Assure that shift report starts on time.
  • Monitor breaks to provide adequate staffing.
  • Determine staffing needs for the next shift and communicate those needs to the Staff Facilitator or Nursing Supervisor.
  • Assign staff to ensure equipment checks are completed: PCX controls, crash cart, refrigerator temps, red books and narcotic counts.
  • Coordinate care in high-risk/emergent situations and keep Nurse Manager/Director/Nursing Supervisor aware of critical patients and any unit issues.
  • Ensure collaboration between staff, physicians, other providers, and departments involved in the care of patients on their unit.
  • Keep lines of communication open.
  • Ensure patient care issues are documented and reported.
  • Assure the appropriate nurse completes a Statement of Concern.
  • Understand and utilize Medical Staff and Kaweah Health's chain of command.
  • Provide employee guidance and alert the Nurse Manager of counseling needs.
  • Provide patient care as needed.
  • Make fiscally responsible decisions regarding staffing and use of patient supplies.
  • Assign patients to infusion chairs and initiate nursing intake (if assigned to Infusion Center).
  • Educate patients on upcoming medications and therapies (if assigned to Infusion Center).
  • Start peripheral IVs or access Central Venous Access Devices (PICC, Port-a-Cath) lines and administer infusions per physician orders (if assigned to Infusion Center).
  • Monitor vital signs and patient status throughout treatment (if assigned to Infusion Center).
  • Perform medication reconciliation and obtain patient history (if assigned to Infusion Center).
  • Administer medications (IV, IM, SQ, PO), IV hydration, blood transfusions, and dressing changes (if assigned to Infusion Center).
  • Complete all infusion documentation, discontinue IVs, and provide discharge instructions (if assigned to Infusion Center).
  • Maintain competency in IV therapy, infusion pumps, and emergency response procedures (if assigned to Infusion Center).
  • Provide telephone triage and patient education on side effects, symptom management, and resources (if assigned to Infusion Center).
  • Communicate and collaborate closely with physicians, inpatient and outpatient nurses, Wound Center staff to coordinate care (if assigned to Wound Center).
  • Function as a case manager for assigned patients with responsibility to plan, implement and evaluate the care for Wound Center patients (if assigned to Wound Center).
  • Ensure patients and families receive comprehensive education related to management of their wounds and underlying health conditions (if assigned to Wound Center).
  • Work with appropriate autonomy in the wound center and hyperbaric setting (if assigned to Wound Center).
  • Function as a resource to staff, management and other health team members regarding nursing practice for wound care (if assigned to Wound Center).
  • Facilitate staff development and education (if assigned to Wound Center).
  • Assist in the development and maintenance of Wound Center policies and procedures (if assigned to Wound Center).
  • Comply with department requirements for continuing education and maintain active participation in hyperbaric related organizations (if assigned to Wound Center).
  • Take part in required Healogics training, competency assessments, and ongoing education as approved by Kaweah Health leadership (if assigned to Wound Center).
  • Support and follow all relevant clinical protocols, operational procedures, documentation standards, and workflows outlined in the Healogics Clinical Policies and Procedures (if assigned to Wound Center).
  • Work in alignment with the program’s performance expectations, quality measures, and operational requirements established for the wound care program (if assigned to Wound Center).
  • Demonstrate the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area.
  • Perform other duties as assigned.
