Requirements

Current, unrestricted Registered Nurse (RN) license
Minimum of one (1) year of nursing experience in a clinical setting
Excellent assessment, communication, and critical thinking skills
Compassionate, patient-centered approach to care
Ability to work independently while collaborating with an interdisciplinary team
Reliable transportation, valid driver's license, and current auto insurance
Willingness to travel throughout the assigned service area

Nice To Haves

Associate or Bachelor's Degree in Nursing
Previous Home Health or Hospice experience
Experience with Homecare Homebase (HCHB)

Responsibilities

Complete comprehensive patient assessments and ongoing evaluations.
Develop, implement, and adjust individualized plans of care.
Educate patients and caregivers on disease management, medications, treatments, and self-care.
Monitor patient progress and identify changes in condition.
Collaborate with physicians and the interdisciplinary care team to coordinate services.
Accurately document clinical findings and care provided.
Advocate for patients while promoting independence and safety at home.
Maintain compliance with all federal, state, and organizational standards, including HIPAA.
