We are currently seeking a full-time or part-time Field RN to support clients throughout Erie County. In this role, you will provide clinical oversight and support to clients in their homes while enjoying a flexible schedule during regular business hours, with occasional on-call responsibilities. Requirements

  • NYS licensed and currently registered as a Registered Professional Nurse.
  • Must have 1 year Home Care experience (this is a regulatory requirement)
  • Thorough knowledge of the scope of practice of professional and paraprofessional healthcare.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to speak effectively before customers/patients or employees of the agency.
  • Excellent organizational writing, communication and interpersonal skills.
  • Ability to apply common sense understanding to carry out instructions furnished in written, verbal or diagram form.
  • Required to participate in on-call rotation (+$7/hour)

Responsibilities

  • Provides medical oversight for clients in their homes- including but not limited to assessments, obtaining MD orders, monthly contacts, and maintaining an appropriate Plan of Care.
  • Communicate with referrers, discharge planners, and physicians to ensure seamless patient care.
  • Develop, implement, and monitor individualized treatment plans, ensuring proper care coordination and medication management.
  • Perform in-home patient assessments, reassessments, and provide intermittent nursing services.
  • Ensure accurate documentation and compliance with agency policies, including patient rights and discharge planning.
  • Supervise and provide in-service education for staff, conducting competency evaluations.
  • Participate in quality improvement programs and policy reviews.
  • Provide 24-hour on-call nursing coverage on a rotating basis.
  • May assist with PCA Training Program as Needed.
