About The Position

We are seeking a compassionate and skilled Registered Nurse to provide high-quality in-home and community-based patient care through scheduled homecare visits. This role focuses on delivering individualized support, assessing patient needs, coordinating care, and promoting positive health outcomes in a patient-centered environment.

Requirements

  • Current and active Registered Nurse licensure in the State of Georgia
  • Strong assessment, critical thinking, and clinical decision-making skills.
  • Ability to communicate effectively and compassionately with patients, families, and colleagues.
  • Excellent organizational skills with the ability to manage multiple visits and priorities.
  • Proficiency in electronic documentation and familiarity with HIPAA and patient confidentiality requirements.
  • Current CPR/First Aid certification or willingness to obtain as required.
  • Valid driver’s license, reliable transportation, and willingness to travel to patient locations as needed.

Nice To Haves

  • Experience in homecare, home health, visiting nurse services, case management, or a related clinical setting preferred.
  • Adaptable and comfortable working independently in varied patient environments.
  • Strong problem-solving skills and attention to detail.
  • Commitment to delivering respectful, patient-centered care.
  • Ability to build trust and maintain professional relationships across diverse populations.

Responsibilities

  • Conduct patient visits to assess health status, monitor progress, and document findings accurately and thoroughly.
  • Develop and follow care plans in collaboration with patients, families, and the broader care team.
  • Educate patients and caregivers on care plans, medications, symptom management, and preventive health practices.
  • Observe for changes in condition, respond appropriately, and escalate concerns to the appropriate clinical contacts.
  • Maintain timely, complete, and compliant clinical documentation.
  • Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
  • Support continuity of care through scheduling, follow-up, and communication with the care team.
  • Promote safety, infection control, and best practices during all patient interactions.
