About The Position
Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Our goal is to deliver quality home health services, in good faith, to those who entrust themselves to our care and to minister to the unique needs of each individual with respect, dignity, and compassion. This Home Health Registered Nurse RN Position is for individuals with a passion for personalized, one-on-one patient care who thrive on building meaningful relationships and empowering patients. The role offers autonomy and flexibility, allowing the nurse to manage their own schedule, prioritize tasks, and make independent decisions regarding patient care. There are opportunities for continuous growth through paid training, scholarships, and specialization. The areas for the Registered Nurse include Bridgeville/Upper St. Clair and Mt. Lebanon/Castle Shannon.
Requirements
- Minimum one year of recent clinical experience as a Registered Nurse recommended.
- Home care experience recommended.
- Adaptability, compassion, and a commitment to patient-centered care.
- Ability to work independently and manage time effectively.
Responsibilities
- Conduct thorough assessments and develop individualized care plans in collaboration with physicians, patients, and families.
- Treatments, IV starts, Chest Tube Management, and Wound care.
- Educating and empowering patients and families on managing their health conditions and building confidence in self-care.
- Utilizing strong clinical assessment skills to monitor patient conditions and identify changes.
