About The Position

This role focuses on providing complete ongoing care for dialysis patients within Skilled Nursing Facilities (SNFs). DaVita operates 6-chair dialysis dens on-site at SNFs, addressing the burden on patients and healthcare costs associated with transporting SNF residents to external dialysis centers. The Registered Nurse (RN) in this position will troubleshoot and make sound decisions for patients' dialysis treatments, occasionally coordinating care with SNF staff. A significant part of the role involves patient and family education. The RN is responsible for independently delivering optimal dialysis care and supervising Dialysis Technicians. This complex service line requires multitasking to meet the needs of the service, patients, and SNF partners, with a strong emphasis on autonomy and critical thinking. The RN will assess, diagnose, and implement complete care plans, looking at the whole patient.

Requirements

  • Previous Chronic Hemodialysis, Acute Dialysis and/or Peritoneal Dialysis (PD) nursing experience.
  • Current Registered Nurse (RN) license in the state of practice.
  • Minimum of 12 months' of RN experience.
  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree.
  • Basic computer skills and proficiency in MS Word and Outlook.

Responsibilities

  • Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
  • Troubleshoot and make sound decisions for patients related to their dialysis treatment.
  • Occasionally partner alongside SNF staff to coordinate care.
  • Conduct impactful and meaningful education to patients and their families in the SNF setting.
  • Independently demonstrate best practice techniques and deliver optimal dialysis care.
  • Supervise Dialysis Technicians who are tasked with delivering tandem care to meet the capacity and efficiency goals of the program.
  • Assess, diagnose, and implement complete care plans as you look at the whole patient.
