About The Position

The Adult Foster Care (AFC) Registered Nurse provides clinical oversight, home-based assessment, and coordinated support services for individuals with medical needs and/or developmental disabilities within the AFC program. This role utilizes a collaborative, team-based approach to ensure high-quality, person-centered care and regulatory compliance. LPNs encouraged to apply as well.

Requirements

  • Current Massachusetts Registered Nurse (RN) licensure OR LPN license.
  • Minimum of 3 year of experience in human services, long-term supports, disability services, or a related program area.
  • Understanding of home- and community-based service models, AFC, Shared Living, or related MassHealth programs.
  • Excellent communication, organizational, and interpersonal skills.
  • Proficiency with Electronic Health Records (EHR), and Microsoft Office Suite.
  • Valid driver’s license and reliable transportation for travel.
  • Ability to complete all required trainings required such as CPR and First Aide

Nice To Haves

  • Experience working in community-based programs.
  • Experience with Medicaid, MassHealth, and state funding systems.
  • Bilingual or multilingual skills.

Responsibilities

  • Participate in the evaluation of potential AFC members and prospective caregivers.
  • Conduct clinical assessments and develop individualized, multidisciplinary care plans.
  • Provide clinical supervision and ongoing support for assigned AFC placements, including monthly home visits and case management.
  • Monitor participant health status on a continuous basis and respond to changes promptly.
  • Provide education to members and caregivers regarding medical concerns, prevention strategies, and wellness practices.
  • Coordinate community medical services and personal care supports for members as needed.
  • Collaborate with primary care providers, community service providers, medical professionals, and internal team members to ensure integrated care.
  • Complete and/or review all required AFC program documentation in accordance with agency standards.
  • Maintain compliance with Kennedy-Donovan Center mission, philosophy, policies, procedures, and quality standards.
  • Ensure adherence to all applicable state and federal funding agency certification requirements and regulations.
  • Maintain certifications and competencies required by the agency and funding sources.
  • Serve as a resource for caregivers, case managers, and interdisciplinary team members regarding clinical needs.
  • Communicate effectively with primary care providers, caregivers, families, and program staff to share updates on participant health and support needs.
  • Participate in professional development activities to enhance clinical skills and maintain licensure.
  • Perform other job-related responsibilities as assigned.
