About The Position

The Hospice Center RN provides comprehensive nursing care to hospice patients with complex symptom-management needs in a six-bed inpatient hospice unit. The RN works collaboratively with physicians, advanced practice providers, social workers, chaplains, hospice aides, and other interdisciplinary team members to promote comfort, dignity, and quality of life for patients and families. The role is full-time on the evening shift, 3:00 p.m. to 11:30 p.m., with an every-other-weekend rotation. The work location is 2404 Valparaiso Street, Valparaiso, Indiana.

Requirements

Current Indiana Registered Nurse (RN) license in good standing.
Graduate of an accredited nursing program.
Current CPR certification.
Ability to work independently and collaboratively within an interdisciplinary team.
Excellent communication and critical thinking skills.

Nice To Haves

Previous hospice, palliative care, oncology, critical care, medical-surgical, ER, ICU, or long-term care experience.
Experience with symptom management and end-of-life care.

Responsibilities

Assess, plan, implement, and evaluate individualized patient care plans.
Manage pain and symptoms using hospice and palliative care best practices.
Administer medications, treatments, and therapies in accordance with physician orders.
Monitor patient condition and communicate changes to the hospice interdisciplinary team.
Provide education and emotional support to patients and their families.
Collaborate with physicians and team members to coordinate patient care.
Maintain accurate and timely documentation in the electronic medical record.
Advocate for patient comfort, dignity, and end-of-life wishes.
Participate in team meetings, care conferences, and quality improvement initiatives.
Provide compassionate support to families throughout the hospice journey.
