About The Position
PruittHealth @ Home is hiring full-time Registered Nurses for home health roles, with special consideration for applicants who live in or can commute to Cherokee, Forsyth, or North Fulton counties in Georgia. A $10,000 sign-on bonus is available for Cherokee and Forsyth county openings only. The bonus is paid through an earn-as-you-go plan and does not require an employment contract; North Fulton openings do not include a sign-on bonus. As a Home Health RN Case Manager, you will provide skilled nursing care in patients’ homes, perform assessments and start-of-care visits, develop care plans, and coordinate with the interdisciplinary care team. You will supervise LPNs and Home Health Aides, educate patients and families, and support continuity, quality, and safety of care. Documentation is completed using an iPad with AI assistance; OASIS experience is a plus. Requirements
- Active, unrestricted Registered Nurse (RN) license.
- Valid driver’s license and reliable transportation.
- CPR certification.
- At least 1 year of RN experience; home health or acute care experience is preferred.

- Experience with aging or adult patient populations.
- OASIS experience.
- Strong organization and time-management skills, including the ability to multitask and prioritize a busy caseload.
- Confidence working with technology and electronic documentation systems.
Responsibilities
