About The Position

Elite Home Health is a Medicare-certified home health agency serving communities across Florida. We are seeking dedicated Registered Nurses who are committed to clinical excellence, compliance, and delivering high-quality patient care in the home setting. This role is for a compassionate, detail-oriented nurse who values autonomy and teamwork.

Requirements

  • Graduate of an accredited School of Nursing (ASN or BSN required).
  • Current Florida Registered Nurse (RN) license in good standing.
  • Current CPR certification required.
  • Valid driver’s license, reliable transportation, and auto insurance.
  • Ability to pass background screening and all compliance requirements.
  • Ability to travel to patient homes within assigned territory.
  • Ability to stand, walk, bend, lift, and assist patients as needed.
  • Ability to perform physical and clinical assessments in home environments.
  • Ability to use electronic documentation systems.
  • Ability to communicate effectively in person, by phone, and electronically.

Nice To Haves

  • Minimum of 1 year nursing experience preferred.
  • Home health, hospice, or acute care experience preferred.

Responsibilities

  • Provide and coordinate skilled nursing care for patients in the home setting.
  • Complete comprehensive assessments.
  • Develop and implement patient care plans.
  • Ensure care is delivered in accordance with physician orders, Medicare Conditions of Participation, state regulations, and agency policies.
  • Serve as a key member of the interdisciplinary team and ensure safe, effective, and compliant care delivery.
  • Perform comprehensive and ongoing patient assessments, including start of care (SOC), recertification, and reassessment visits.
  • Complete OASIS assessments as required.
  • Develop, implement, and update individualized patient care plans.
  • Evaluate patient response to care and adjust interventions as needed.
  • Ensure physician orders are appropriate and consistent with the patient’s clinical condition and care plan, and communicate discrepancies.
  • Provide skilled nursing services in accordance with physician orders and care plans.
  • Coordinate care with physicians, therapists, and interdisciplinary team members.
  • Ensure continuity of care across all services provided.
  • Monitor patient condition and identify changes in physical, emotional, or functional status.
  • Communicate significant changes promptly to physician and care team.
  • Implement appropriate interventions within scope of practice.
  • Complete accurate, timely clinical documentation in the EMR system.
  • Ensure documentation supports medical necessity and compliance requirements.
  • Maintain adherence to Medicare Conditions of Participation and agency policies.
  • Complete OASIS assessments accurately and within required timeframes.
  • Provide direction and supervision to LPNs and CNAs as applicable.
  • Participate in interdisciplinary team meetings and care coordination.
  • Support clinical education and guidance for field staff.
