About The Position

Elite Home Health is a Medicare-certified home health agency serving communities across Florida. We are seeking dedicated Registered Nurses who are committed to clinical excellence, compliance, and delivering high-quality patient care in the home setting. This role is for a compassionate, detail-oriented nurse who values autonomy and teamwork.

Requirements

Graduate of an accredited School of Nursing (ASN or BSN required).
Current Florida Registered Nurse (RN) license in good standing.
Current CPR certification required.
Valid driver’s license, reliable transportation, and auto insurance.
Ability to pass background screening and all compliance requirements.
Ability to travel to patient homes within assigned territory.
Ability to stand, walk, bend, lift, and assist patients as needed.
Ability to perform physical and clinical assessments in home environments.
Ability to use electronic documentation systems.
Ability to communicate effectively in person, by phone, and electronically.

Nice To Haves

Minimum of 1 year nursing experience preferred.
Home health, hospice, or acute care experience preferred.

Responsibilities

Provide and coordinate skilled nursing care for patients in the home setting.
Complete comprehensive assessments.
Develop and implement patient care plans.
Ensure care is delivered in accordance with physician orders, Medicare Conditions of Participation, state regulations, and agency policies.
Serve as a key member of the interdisciplinary team and ensure safe, effective, and compliant care delivery.
Perform comprehensive and ongoing patient assessments, including start of care (SOC), recertification, and reassessment visits.
Complete OASIS assessments as required.
Develop, implement, and update individualized patient care plans.
Evaluate patient response to care and adjust interventions as needed.
Ensure physician orders are appropriate and consistent with the patient’s clinical condition and care plan, and communicate discrepancies.
Provide skilled nursing services in accordance with physician orders and care plans.
Coordinate care with physicians, therapists, and interdisciplinary team members.
Ensure continuity of care across all services provided.
Monitor patient condition and identify changes in physical, emotional, or functional status.
Communicate significant changes promptly to physician and care team.
Implement appropriate interventions within scope of practice.
Complete accurate, timely clinical documentation in the EMR system.
Ensure documentation supports medical necessity and compliance requirements.
Maintain adherence to Medicare Conditions of Participation and agency policies.
Complete OASIS assessments accurately and within required timeframes.
Provide direction and supervision to LPNs and CNAs as applicable.
Participate in interdisciplinary team meetings and care coordination.
Support clinical education and guidance for field staff.
