About The Position

This is a Registered Nurse (RN) position focused on Home Health in South Dennis, Massachusetts. The role involves maintaining HHRG values, determining visit frequency, achieving patient outcomes, referring patients to community resources, assessing patient needs, developing and revising care plans, participating in case conferences, supervising Home Health Aides, communicating changes to physicians and care teams, completing admissions and documentation, obtaining physician orders, planning discharges, and completing discharge documentation. The position requires an active RN license in Massachusetts and BLS certification.

Requirements

  • Active Registered Nurse (RN) license in the state of Massachusetts
  • Basic Life Support (BLS) certification (American Heart Association)
  • Minimum of 1 year of nursing experience required OR 6 months of nursing experience with a Bachelor of Science in Nursing (BSN)

  • Experience in home health settings is preferred.
  • Strong clinical assessment and documentation skills are essential.

Responsibilities

  • Maintain annual HHRG values in alignment with national standards.
  • Utilize HHRG metrics to determine visit frequency and appropriate interdisciplinary care.
  • Achieve annual patient outcomes that meet national benchmarks.
  • Refer patients to community resources when appropriate.
  • Assess patient clinical needs and develop, implement, and revise individualized care plans.
  • Address all identified problems in the plan of care and document accordingly.
  • Participate in interdisciplinary case conferences and evaluate care effectiveness.
  • Provide instruction and documented supervision of Home Health Aide care plans every 14 days.
  • Communicate significant patient condition changes to physicians and care team members.
  • Complete admissions and required documentation in a timely manner.
  • Maintain clear, concise, and accurate clinical documentation reflecting skilled care provided.
  • Obtain and clarify valid physician orders for all care services delivered.
  • Plan and coordinate patient discharges, including communication with patient and physician.
  • Complete all discharge documentation and summaries accurately.
