About The Position
Community Care, Inc. is looking for a compassionate and dedicated RN to provide home health care to our clients in the Westmoreland and Fayette County area. Requirements
- Current Registered Nurse (RN) license in the state of Pennsylvania.
- Graduate of an accredited nursing program
- Minimum 1 year of clinical nursing experience
- CPR certification (required)
- Reliable transportation and a valid driver's license
- A caring heart, strong independence, and a passion for pediatric care
Nice To Haves
- Home Health experience preferred
Responsibilities
- Conduct skilled nursing visits in patients' homes (assessment, reassessment, medication administration, IV therapy, and treatments)
- Develop and implement individualized care plans
- Educate patients and caregivers on managing conditions and medications
- Collaborate with a multidisciplinary team to ensure continuity of care
- Document visits and patient progress accurately and timely
