About The Position
The Hospice Center RN provides comprehensive nursing care to hospice patients with complex symptom-management needs in a 6-bed inpatient hospice unit in Valparaiso, Indiana. The RN delivers patient-centered end-of-life care focused on comfort, dignity, and quality of life, while supporting patients and their families. The role works collaboratively with physicians, advanced practice providers, social workers, chaplains, hospice aides, and other interdisciplinary team members. This is a full-time day-shift position scheduled from 7:00 a.m. to 3:30 p.m. It requires an every-other-weekend rotation, with holidays as scheduled.
Requirements
- Current Indiana Registered Nurse (RN) license in good standing.
- Graduate of an accredited nursing program.
- Current CPR certification.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Excellent communication and critical thinking skills.
Nice To Haves
- Previous hospice, palliative care, oncology, critical care, medical-surgical, ER, ICU, or long-term care experience.
- Experience with symptom management and end-of-life care.
Responsibilities
- Assess, plan, implement, and evaluate individualized patient care plans.
- Manage pain and symptoms using hospice and palliative care best practices.
- Administer medications, treatments, and therapies in accordance with physician orders.
- Monitor patient condition and communicate changes to the hospice interdisciplinary team.
- Provide education and emotional support to patients and their families.
- Collaborate with physicians and team members to coordinate patient care.
- Maintain accurate and timely documentation in the electronic medical record.
- Advocate for patient comfort, dignity, and end-of-life wishes.
- Participate in team meetings, care conferences, and quality improvement initiatives.
- Provide compassionate support to families throughout the hospice journey.
