About The Position

As a Registered Nurse (RN) at our post-acute care facility, you will play a key role in delivering compassionate, patient-centered nursing care to individuals in both short-term rehabilitation and long-term care settings. You will assess, plan, implement, and evaluate individualized care plans to support recovery, improve health outcomes, and ensure the ongoing well-being of each resident.

Requirements

  • Current and valid Registered Nurse (RN) license in the state of practice.
  • Minimum of 2 years of nursing experience, preferably in a skilled nursing, rehab, or post-acute care setting.
  • CPR and Basic Life Support (BLS) certification.
  • Strong clinical judgment, organizational, and critical thinking skills.
  • Excellent communication, patient education, and teamwork abilities.
  • Familiarity with regulatory guidelines related to both SNF and rehabilitation care (CMS, state, etc.).

Responsibilities

  • Perform thorough assessments of residents and short-term patients to identify physical, emotional, and psychosocial needs.
  • Develop and implement individualized care plans tailored to recovery goals or long-term wellness needs.
  • Administer medications, IVs, and prescribed treatments accurately and safely.
  • Monitor patient responses and adjust care accordingly in coordination with the healthcare team.
  • Collaborate with therapy staff (PT, OT, Speech) to support short-term rehab patients in achieving recovery goals and safe discharge plans.
  • Monitor progress of rehab patients and advocate for changes in care when necessary.
  • Provide specialized wound care, especially for post-operative or rehab patients.
  • Ensure proper documentation and follow-up.
  • Monitor vital signs and health status; identify and respond to changes in condition.
  • Accurately chart assessments, interventions, and outcomes in compliance with regulatory standards.
  • Educate patients and families on health conditions, medication regimens, and post-discharge care.
  • Support safe transitions to home or alternate levels of care, particularly for short-term rehab patients.
  • Work closely with physicians, therapists, social services, and other interdisciplinary team members to ensure continuity of care across SNF and rehab units.
  • Follow infection prevention protocols and contribute to a clean, safe care environment.
  • Provide timely and appropriate emergency interventions in accordance with facility policy.
  • Participate in ongoing performance improvement and compliance initiatives to ensure excellence in care delivery.
