This role involves providing high-quality nursing care to patients in their homes, developing and implementing individualized patient care plans, administering medications and treatments as prescribed, and completing and submitting OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition reports within agency-defined timeframes.
Requirements
- Valid Registered Nurse license in Massachusetts
- 1-2 years of experience as an Registered Nurse
- 1 year of experience in home health care required
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Reliable transportation and willingness to travel within the service area.
Responsibilities
- Provide high-quality nursing care to patients in their homes
- Develop and implement individualized patient care plans
- Administer medications and treatments as prescribed
- Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
