About The Position

The Registered Nurse (RN) oversees all medical aspects of the person supported health. This includes ensuring regular face-to-face contact, scheduling necessary medical appointments, completing nursing assessments, and ensuring adequate medication supply. The RN reviews medical summaries, acts as an advocate for the person supported during medical care, and represents the agency in professional settings. They also provide coverage for other sites as needed and visit individuals during hospitalizations or rehab admissions.

Requirements

  • Valid N.Y.S Driver's License

Responsibilities

  • Oversees all medical aspects of the person supported health, including weekly face-to-face contact, scheduling medical appointments, and completing nursing assessments.
  • Ensures an adequate supply of medication and that persons supported are receiving prescribed medications, treatments, and diets.
  • Reviews all routine, pre- and post-surgical, and Urgent Care/ER visit summaries, documenting findings and recommendations.
  • Serves as an advocate for the person supported during medical care with the Prescriber.
  • Maintains a high level of professionalism, leads professional settings such as audits, and participates in conferences representing the agency.
  • Visits people supported during hospitalizations or nursing rehab admissions to ensure healthcare needs are met.
  • Provides RN Coverage of other I/DD sites as deemed necessary.
  • Actively participates as a member of the person supported program planning team, ensuring health and medical issues are discussed and a plan of care is developed.
  • Maintains all mandated medical records, including nursing notes, medical visit summaries, discharge summaries, prescriber's orders, and Plans of Nursing Services (PONS).
  • Completes all medical reports for the person supported in a timely manner and participates in meetings to provide medical updates.
  • Responsible for the oversight of person supported charts, including MAR/Treatment Records, updated prescriptions, BM Charts, Medical Books, Flowsheets, and PONS.
  • Participates in the internal auditing process utilizing the Chart Audit Tool on a quarterly basis.
  • In-services staff on general health issues and person supported specific needs.
  • Trains AMAP staff on responsibilities regarding medication administration.
  • Ensures that health and safety procedures are being followed according to State and Federal regulations.
  • Triage health concerns of individuals via telephone and provides directives to staff.
  • Directs staff to utilize Telehealth services when deemed necessary.
  • Supervises the AMAP's dispensing of medication and treatments.
  • Ensures that medication errors are reported and AMAPs are trained on prevention.
  • Maintains a standard of high quality of work and productivity.
  • Has overall knowledge of the person supported program plans, medical status, and day program.
  • Follows through on issues in a timely manner.
  • Demonstrates good judgment and decision-making.
  • Is a good role model for both staff and person supported.
  • Has a good attitude and team spirit.
  • Is flexible and takes initiative.
