Registered Nurse (RN) - On-Call Community Hospice - Per Diem

About The Position

The Community Hospice is seeking a compassionate, dedicated per diem Registered Nurse for its After-Hours Hospice Care Team in the Capital/Saratoga region. The nurse provides critical care and support to hospice patients and families in homes and nursing facilities during evening, overnight, or weekend hours. The role focuses on skilled nursing assessment, symptom management, comfort-focused care, and compassionate end-of-life support, helping patients remain comfortable in their preferred care setting. The nurse collaborates with an interdisciplinary hospice team to promote continuity of care, comfort, dignity, and quality of life. The organization provides a thorough orientation, specialized hospice and end-of-life training, preceptors, ongoing mentorship, and a supportive team environment. A laptop is supplied for meetings and charting, and mileage is reimbursed. The organization also provides necessary protective equipment and emphasizes staff safety.

Requirements

Valid New York State RN license.
One to two years of previous RN experience.
Ability to learn and use an electronic medical record.
Clean driver's license and reliable vehicle.
Strong interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers.

Nice To Haves

Homecare and/or hospice experience is preferred.
Passion for compassionate care and eagerness to learn.

Responsibilities

Provide after-hours nursing visits to hospice patients in home and facility settings.
Assess and manage symptoms and address urgent patient and family needs.
Provide education, guidance, and emotional support to patients and caregivers.
Coordinate care with the interdisciplinary hospice team.
Communicate changes in patient condition and ensure appropriate follow-up.
Maintain accurate and timely clinical documentation.
