About The Position
Concordia Visiting Nurses is seeking a Registered Nurse (RN) with a passion for personalized, one-on-one patient care. As a Home Health RN, you will be responsible for conducting thorough assessments, developing individualized care plans in collaboration with physicians, patients, and families, and providing treatments such as IV starts, Chest Tube Management, and Wound care. You will also focus on educating and empowering patients and families on managing their health conditions and building confidence in self-care, utilizing strong clinical assessment skills to monitor patient conditions and identify changes.
Requirements
- Minimum one year of recent clinical experience as a Registered Nurse recommended.
- Home care experience recommended.
- Adaptability, compassion, and a commitment to patient-centered care.
- Ability to work independently and manage time effectively.
Responsibilities

- Treatments, IV starts, Chest Tube Management, and Wound care.
