About The Position
Home Care Now is seeking a compassionate and clinically strong Home Health RN to provide one-on-one care in someone's home. This role is ideal for a nurse who values autonomy, patient relationships, and the ability to deliver skilled care in a home-based setting rather than a facility environment.
Requirements
- Active RN license in the state of employment
- Current CPR or BLS certification
- Level 2 Background
- Vent, teach gtube experience
- Strong assessment and communication skills
- Ability to work independently in a home-based care setting
Nice To Haves
- 1 year Home health experience preferred
Responsibilities
- Assess patient condition, vital signs, and response to treatment
- Administer medications and perform wound care as ordered
- Educate patients and families on care plans and home health best practices
- Coordinate with physicians, family members, and the broader care team
- Complete accurate, timely documentation for each visit
