Requirements

  • Graduate of an accredited nursing program.
  • Current, unrestricted Pennsylvania Registered Nurse (RN) license and/or applicable multistate (compact) RN privilege, maintained in good standing.
  • Minimum of two years of clinical experience as a Registered Nurse.
  • Current BLS certification.
  • Naloxone training/certificate of completion as required by FFH.
  • Knowledge of computer software applications (Microsoft Office Applications, others)
  • Knowledge of grammar, spelling, and punctuation as it relates to correspondence necessary to perform essential job functions
  • Knowledge of basic arithmetic to make calculations, balance and reconcile figures necessary to perform essential job functions
  • Excellent customer service skills
  • Skill in operating office equipment
  • Ability to critically think which means the ability to think clearly and rationally about what to do or what to believe; understand logical connections between ideas; and identify, construct, and evaluate arguments

Nice To Haves

  • Bachelor of Science in Nursing (BSN).
  • Ambulatory and/or primary care nursing experience.
  • Previous telephone and/or clinical triage experience.
  • Previous Epic electronic health record experience.

Responsibilities

  • Provides clinical triage for patients presenting with acute, new, worsening, or time-sensitive symptoms, including walk-in patients and patients contacting the practice for clinical guidance
  • Utilizes clinical judgment, established protocols, standing orders, and evidence-based guidelines to determine the appropriate level and urgency of care.
  • Identifies patients requiring immediate intervention and initiates appropriate emergency response procedures, including activation of emergency medical services when indicated.
  • Reinforces provider-established plans of care and ensures patients understand follow-up instructions and parameters for seeking additional care.
  • Documents patient assessments, interventions, communications, education, and disposition accurately and timely in the electronic health record.
  • Maintains complete and accurate clinical documentation consistent with organizational standards and regulatory requirements.
  • Uses appropriate clinical escalation processes when patient needs exceed the RN's scope of practice or require provider intervention.
  • Maintains current knowledge of clinical standards, evidence-based practices, organizational policies, and applicable nursing regulations.
  • Maintains patient confidentiality and complies with applicable privacy, regulatory, professional, and organizational requirements.
  • Serves as the primary clinical resource for patients presenting to the practice without a scheduled appointment who require clinical assessment or assistance.
  • Performs timely assessment of walk-in patients to determine the urgency of the patient's condition and the most appropriate next step in care.
  • Supports safe and efficient patient flow while maintaining appropriate clinical prioritization and patient safety.
  • Provides clinical support to physicians and APPs to promote safe, efficient, and coordinated patient care.
  • Communicates relevant assessment findings and clinical information to providers in a timely, concise, and organized manner.
  • Serves as a clinical resource to medical assistants, LPNs, and other members of the care team.
  • Collaborates with the interdisciplinary care team to address patient needs and promote continuity of care.
  • Reviews and follows up on clinical information, including diagnostic results and other patient-related information, in accordance with established workflows and provider direction.
  • Identifies barriers to care and works collaboratively with the care team to facilitate appropriate resources and follow-up.
  • Monitors patients requiring ongoing clinical follow-up and communicates changes or concerns to the appropriate provider.
  • Promotes a culture of patient safety through appropriate assessment, clinical escalation, communication, and adherence to established protocols.
  • Recognizes potential patient safety concerns and takes timely action to mitigate risk.
  • Participates in quality improvement, patient safety, and clinical process improvement initiatives.
  • Identifies opportunities to improve triage, access, patient flow, care coordination, and clinical workflows.
  • Participates in the development, implementation, and evaluation of clinical workflows, protocols, and practice standards as appropriate.
