Weekend Registered Nurse - Home Visits
About The Position
Valley Health at Home by BAYADA is seeking a Weekend Registered Nurse (RN) to perform home health visits with adult and senior patients throughout the Las Vegas valley. This role offers per diem, part-time, or full-time scheduling options. A current or recent experience in a Medicare Certified Home Health setting with OASIS documentation (especially Start of Care) is required. As a home health RN, you will be a vital part of a multi-disciplinary healthcare team, providing rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care at home.
Requirements
- Current nursing license in good standing in the state of Nevada.
- Graduation from an accredited and approved nursing program.
- Current or recent experience in a Medicare Certified Home Health setting with OASIS documentation (especially Start of Care).

Responsibilities
- Perform head-to-toe assessments and identify subtle changes in patient condition.
- Utilize experience with acute and post-acute care, including CHF, COPD, wound care, IV therapy, and post-surgical care.
- Independently triage and manage urgent clinical situations in the home setting.
- Perform medication reconciliation, teaching, and safety monitoring.
- Conduct comprehensive wound assessments, care, and documentation.
