About The Position
DaVita is seeking an organized, self-directed Registered Nurse to manage peritoneal dialysis (PD) patients. The role combines clinical care, patient education, and case management. The nurse educates patients and caregivers in the clinic so they can perform dialysis independently at home, and supports patients through assessments, care planning, treatment monitoring, home visits, and on-call assistance. The position is scheduled Monday through Friday in 8-hour shifts, with weekends and holidays off. Flexible scheduling is available to manage the nurse’s own caseload.
Requirements
- Current RN license in the state of practice.
- Valid driver’s license, insurance, and reliable transportation.
- Associate Degree in Nursing (ADN) required.
- Basic computer skills, including MS Word and Outlook.
Nice To Haves
- Bachelor of Science in Nursing (BSN).
- Experience with dialysis, including PD, HHD, chronic, or acute dialysis.
- Patient education or teaching background.
- Case management or telehealth experience.
- CNN or CDN certification.
Responsibilities
- Train patients and caregivers on peritoneal dialysis procedures and self-care.
- Manage a caseload of PD patients, including conducting assessments and developing care plans.
- Monitor treatment progress, conduct home visits, and provide on-call support.
- Collaborate with interdisciplinary teams to support safe, high-quality outcomes.
