About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services to help people receive care at home. In this role, the Registered Nurse serves as a Nurse Case Manager, guiding patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, the nurse helps ensure patients receive the appropriate level of care and provides education, coaching, and treatment-decision support in collaboration with a Care Advocate and Social Worker. The main coverage area is ZIP code 37803. The role includes both per-visit-point and hourly compensation, depending on the work performed.
Requirements
- Current RN license specific to the state or states assigned.
- At least 1 year of clinical experience as a Registered Nurse.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other disciplines based on patients’ identified needs, or document the rationale for not making a referral.
- Promote patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
- Supervise LPNs and HHAs.
- Complete documentation accurately and on time.
- Regularly communicate patient progress to the clinical manager and care team.
- Plan and provide staff education.
- Perform assigned on-call responsibilities and provide on-call services to patients and families.
- Participate in clinical development and continuing education programs.
- Perform other duties as assigned.
