About The Position
Catholic Health Home Care is hiring a Registered Nurse to provide home health care to patients across a variety of populations and potentially complex home situations. The role includes case management, patient assessment and care planning, skilled nursing procedures, prescribed treatments, and patient and family education. Nurses collaborate with physicians and a supportive care team. The position offers 8-hour shifts from 8:30 AM to 4:30 PM, with rotating weekends. Per-visit staff must attend a full week of weekday orientation from 8:30 AM to 4:30 PM and work one shift per pay period.
Requirements
- Current New York State RN license.
- At least one year of recent hands-on experience as a Registered Nurse.
- Basic Life Support certification.
- Valid New York State driver’s license and reliable transportation.
Responsibilities
- Case manage, assess, plan, develop, and implement comprehensive home health care plans for patients.
- Measure vital signs; administer IVs; change dressings; and clean wounds as needed.
- Provide skilled nursing care, preventive and rehabilitative procedures, and prescribed treatments to varied patient populations in potentially complex home situations.
- Collaborate with physicians, therapy, social services, pastoral care, home health aides, and other members of the patient care team for assigned caseloads.
- Provide patient and family education on disease management and health promotion as directed by the physician’s plan of treatment.
