About The Position

UPLIFT HOMECARE, LLC offers compassionate, professional in-home care services tailored to meet the needs of seniors and veterans. We are seeking a highly skilled and dedicated Registered Nurse to join our team. The ideal candidate will possess extensive clinical expertise, including experience in various healthcare settings such as hospitals, long-term care facilities, and outpatient clinics. As a Registered Nurse with UPLIFT HOMECARE, LLC, you will deliver exceptional patient care, coordinate treatment plans, and support our mission to enhance quality of life through personalized healthcare services.

Requirements

  • Valid RN license with current registration in the state of Florida.
  • Proficiency in using EMR/EHR systems such as Cerner or Axxess for documentation purposes.
  • Oasis Home Health Care 1 year of experience.

  • Excellent communication skills coupled with a compassionate approach to patient care.
  • Home health: 1 year (Required)
  • CPR Certification (Required)
  • Florida RN License (Required)
  • Drivers License (Required)

Nice To Haves

  • Proven experience in acute care settings such as ICU or Level I Trauma Centers is highly desirable.
  • Experience working with diverse populations including pediatrics, geriatrics, individuals with developmental disabilities, or those requiring hospice & palliative medicine.

Responsibilities

  • Participates in ongoing interdisciplinary assessment of the patient.
  • Completes initial and ongoing comprehensive assessments of the patient’s needs at appropriate time points as assigned.
  • Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Contributes to the patient’s plan of care/service plan.
  • Administer medications, injections, and treatments following established protocols and safety guidelines.
  • Monitor vital signs, perform physical examinations, and evaluate patient responses to interventions.
  • Utilize EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems to document patient information accurately and efficiently.
  • Provide education on health management and medication administration to patients and their families.
  • Collaborate with interdisciplinary teams including physicians, therapists, and case managers to ensure coordinated care delivery.
  • Maintain strict adherence to HIPAA regulations to protect patient confidentiality and privacy.
  • Assist with specialized procedures such as IV infusion, suturing, catheterization, and sterile processing when required.
  • Uses clinical decision making to efficiently and effectively manage individual patients.
  • Participates in Quality Assessment and Performance Improvement (QAPI) program.
  • Participates in in-services or training, as requested.
