About The Position

Our mission spans the U.S. We are looking for a Per Diem Registered Nurse for our team in Oregon. This is a remote role with required fieldwork and the possibility of an occasional commute to our Portland, OR or Salem, OR office. CANDIDATES MUST RESIDE IN OREGON. Personal Care Services is a caregiving arrangement where a patient has a principal caregiver providing daily support based on their needs. The goal is to provide necessary care while fostering patient independence in their home, offering a range of care options as needs evolve. This service provides alternatives to nursing facility-level care while preserving patient dignity, self-respect, and privacy. Structured Family Caregiving is a caregiving arrangement where a patient lives with a principal caregiver providing daily support based on their needs. The goal is to provide necessary care while fostering patient independence in a home environment, offering a range of care options as needs evolve. This service provides alternatives to nursing facility-level care for individuals meeting nursing facility-level care needs, while preserving dignity, self-respect, and privacy. The Registered Nurse will be a vital part of administering and ensuring the efficient delivery of services, oversight of the Personal Care Attendants (PCA), and compliance with regulatory requirements. We are seeking an individual with a nursing background with elders or adults with disabilities, as well as experience managing PCAs.

Requirements

  • Valid and active RN License from the Oregon Board of Nursing
  • 2 years of recent work experience in direct care of elders or adults with disabilities
  • Previous experience in a management role
  • Strong communication and interpersonal skills
  • Problem-solving and decision-making abilities
  • Organizational and multitasking skills
  • Comfortable wearing multiple hats; helping where needed
  • Excellent communication, critical thinking, and organizational skills
  • Self-motivated
  • Able to thrive in a fast-paced start-up environment

Responsibilities

  • Ensure compliance with all contractual guidelines and federal and state regulations governing Medicaid and the Personal Care Services program
  • Ensure efficient day-to-day PCA operations by analyzing, monitoring, and optimizing workflows, processes, and procedures. Identify areas for improvement and implement solutions to enhance productivity
  • Providing oversight to PCS/SFC caregivers in conjunction with the Care Manager
  • Conducting initial home visits to define the PCA’s tasks and develop activity plans
  • Performing regular evaluations every 60 days to monitor the PCA’s performance, consumer satisfaction, and adherence to the activity plan
  • Respects client and employee rights and privacy, ensures the security of protected information, practices in an ethical manner and is compliant to the agency’s compliance program and privacy policy
